Have you watched your rack to see exactly what is going on? On mine it appears that the rack is pushed upwards while turning and its basically ripping/punching a hole through the top bushing flange over time.
I'm thinking maybe making the flanges a little thicker and a little wider for the top bushing might help make them last longer if you are having the same problem.
I also bent my OEM bolts and drilled out my steel sleeves larger for a 1/2" bolt, but if we're going to try something custom I would just make the ID of the delrin bushing 1/2" and just get rid of the sleeves.
If you're still offering to make a set, lets say the following dimensions:
Total Length - 1.075
Flange thickness - .156 (for the 2 lower bushings)
Flange thickness - .250 (for the 2 top bushings)
ID - 1/2"
OD - 1.055
Flange width - 1.75 (will require custom fitting prior to install to clear tubing/adjacent interference)

It's not too much of a difference between a 1/2" bolt and the OD of the metal sleeve.@Tetten why get rid of the sleeves? They will increase the surface area, and stiffen up the rack more without it being metal on metal.
If you consider the fact you were cracking metal sleeves with your steering, you should consider how much force is being applied to your bushings, and bolts. I’d say keep the sleeves and take advantage of the larger surface area and the hard surface for the friction to slide against instead of the bolt constantly sliding on the delrin. I cracked a couple of my sleeves at the corner with the flange. I welded the corner, but then they wouldn't fit in the poly bushing....so then I did a half assed countersink in the bushing so the weld would fit in, not to mention the god awful noise they make when the sleeves contact each other when the bushing flex under load. ....so yeah, they've been nothing but problems for me.

Yeah that shit is annoying. We gotta figure out why that is happening. it's called audio float. We don't know why it happens. I think @Trevor Knopp is going to have to spend a little more time editing to figure out where the audio floats and try to get things back in sync.Good episode, but some where around the halfway point the audio looses synch and you guys are talking over each other.

What a clusterfuck that episode was to record lol. Trevor is gonna have some fun editing that one.
The software we use ends the recording if you get an incoming call. It's stupid but it's just how it works. Well my dumbass forgot to put my phone on do not disturb and I got an incoming call halfway through my rant. So we have to start a new recording where we left off. @eimkeith had the same thing happen to him a few minutes later because his DND mode was on a schedule and automatically turned off in the middle of the recording. So we had to start again for the third time but we had some technical difficulties getting all of us back on. Then like a minute before the podcast was over, Keith dropped his phone and ended his recording...
Overall I think this will be a great episode as long as we can get everything pieced together well.
